This appears to be a mark sense card from Remington Rand, but I haven't been able to confirm that. Mark sense cards had a maximum of 40 columns to allow space between marks, which is the case for this one (according to the numbers just below the first row; the numbers across the bottom are for a standard 90-column card). The company name "Remington Rand Univac" shown on the card existed from 1950 to 1955.
